Partners in Care, in partnership with Office of Housing and Community Development (OHCD), are providing a HUD compliant database (CHMIS) for tracking the needs and services across Hawai'I Island to support all providers working to solve and prevent houselessness.
CHMIS MISSION
In the spirit of PIC’s mission to eliminate homelessness through open and inclusive participation in a coordinated response system, CHMIS seeks to provide an accessible, user-friendly database for participating service providers and administrators to better coordinate care and be data-driven in planning, advocacy, and continuity of services for those without housing security on Hawai'i Island.

Provider Forms
All CHMIS Participating Providers need to complete a participation agreement prior to gaining access.
All clients having PII entered into CHMIS must understand and consent to having their data collected.

CHMIS USER Forms

Creating a new profile for your client is easy! This universal form has all the current client details to help identify your client and prevent duplication across programs. This is the first step to enrollment of clients into your program.
Fillable Profile PDF -
This standardized form can be used to collect client data for enrollment into a CHMIS participating program. All fields on this form match what is currently in CHMIS. Client profile data, combined with enrollment details, help to better understand community need and show the hard work being done.
Fillable Program Intake PDF -
Use this form to help track clients you are providing regular outreach services to.

Use this for completing a service-based enrollment (for “services only” projects in CHMIS).

Standardized form for capturing a current living situation of a client during the time a client is enrolled in a program.

The CQA is used to help capture needs and vulnerability of the client to inform referral to resources.
